    Abstract
    Evaluation form for employee work performance completed by employees’ immediate supervisors. The appraisal process involves assessing the employee’s work performance during the review period relative to specific indicators and an overall rating. This scale is applied to the following performance indicators: Work habits, Job knowledge, Communications, Attendance/Punctuality, Teamwork, Productivity, and Customer service.
